If the $0.49 balance is less than the cost of the cheapest item you can buy, you’ll need to contact Apple Support. Apple says they can help clear a small remaining balance when it can’t be spent normally.

Once the balance is zero, you should be able to continue changing your Apple Account country or region, assuming there aren’t any other subscriptions or pending purchases blocking the change.
