Urgent: Unable to Complete Apple Developer Program Enrollment for Over a Month

I hope you are doing well.
I have been trying to enroll in the Apple Developer Program for about a month, but I keep encountering the same issue. I have attempted the enrollment using three different Apple IDs, and on different devices — via the website, my iPhone, and my Mac — but the issue persists.
When I reach the document submission step, I am redirected to a page displaying my email, first name, last name, and address. However, the first name and last name fields are empty and cannot be edited. It seems that these fields should be automatically populated from my Apple ID information, but they remain blank. After completing the remaining address fields and submitting the form, I receive the following error message: "An Error Occurred."
I have already tried contacting support several times, but unfortunately, I have not yet received any response. I would sincerely appreciate if someone could review my case and assist me in completing my enrollment.
